I need a copy of hcfa 1500?

My health insurance carrier is wanting my physical therapy claims submitted on something called a hcfa 1500. My PT provider is out of network and says he doesn't do forms. It's been a huge ordeal and insurance company won't budge on reimbursing me until it's sent to them how they want it. So I've found out from research what all I needed, and PT provider gave me a diagnosis code, cpt codes and their tax id#. So I am trying to figure out where to get one of these forms, and of course insurance won't provide me with one. I found some online places, but they want $30 to buy a box of 500! I only need 2 for crying out loud! I asked my regular physician if they could just copy me one off and they said no, they don't do that. HELP!
